Our professional. reliable. qualified Physical Therapists (PT), Occupational Therapists (OT), and Speech Therapists (ST) assist patients to improve performing daily activities and return to their prior level of activities.

Laboratory and X-Ray Imaging

Certain laboratory tests, such as blood and urine tests, can be performed in the comfort of the patient's home. In addition, mobile X-ray and Ultrasound can be performed also at home.

Skilled Nursing

Our Registered Nurses (RN) provide individual care and case management for medically prescribed treatments and establish a plan of treatment based on and approved by a Medical Doctor.

Our Licensed Vocational Nurses (LVN/LPN) provide individual care following the plan of treatment under supervision of our Registered Nurses.

Medical Social Services and Certified Home Healthcare

Our Licensed Clinical Social Workers (LCSW) assist patients and their family to identify community resources that will help patients remain safe at home.

Our Certified Home Health Aides (CHHA) provide personal care, bathing, and aide in the overall improvement of patients' quality of life.

Wound Care Specialists and Podiatrist

We use Omni Wound Specialist and Wound Care Surgeons team of state licensed physicians, surgeons, physician assistants, and CWCN nurses to provide home visits. We also provide Podiatrists who visit patients at home.

Healthcare Services

Our staff in consultation with a Medical Doctor establish a Plan of Treatment that may include Monitoring the general Health of the Patient, Wound Care; including but not limited to Wound Vac, diabetic ulcers, pressure ulcers, Diabetic Care, Medication Administration and Managment, Pain Management, Colostomy Care, IV Therapy; including but not limited to Start IV, Access PORT-A-Cath, PICC line maintenance, TPN, IVIG.

Our services is not limited to that listed above
